PROGRAM SUMMARY

Social Determinants of Health (SDoH) can impact an individual’s overall health by as much as 80%. SDoH include factors such as literacy, housing, nutrition or safety issues and can be identified through the assignment of Z codes if they are documented and then coded. The Joint Commission (LD.04.03.08), CMS, NCQA and other regulators have adopted requirements for healthcare providers to address health disparities. To achieve compliance, you must capture, analyze and benchmark your data. 

This Program will highlight the concepts, best practices and current trends related to Social Determinants of Health and how clinical documentation and coding practices can positively impact health.
