Postpartum Transitional Care for Birthing People with SUD: The Role of the Social Worker and Community Partners

These offerings are part of a series of educational opportunities for clinicians and care team leadership engaged in the priorities of the New Jersey Perinatal Quality Collaborative’s support of hospitals’ efforts to implement the AIM patient safety bundle focused on caring for pregnant and postpartum people with substance use disorder.

This important course is complimentary, thanks to funding support from the Centers for Disease Control and Prevention through the N.J. Perinatal Quality Collaborative. It provides credits in continuing medical education and continuing nursing education.

Learning Outcomes for this Session:
At the completion of the course, participants will be able to:

  • Describe the role of the social worker when a physician or nurse requests a referral for a postpartum client with a SUD;
  • Examine the resources available for birthing people;
  • Discuss the Maternal Wrap Around Program and its outcomes to date.
